We are so glad you are here. The YRDSB Transition Hub is a space created to support all students and their families during times of change, with specific information and strategies to support students with special education needs.  Here, you’ll find helpful information, clear steps and strategies to support transitions.

There are two ways to access transition resources:

  • Explore some of the transitions and supports available by category.

Every child’s journey is different. This hub is here to support your family as you explore what comes next for your child.

What are Transitions?

Transitions are the moments of change that students experience every day. They can be both big and small. They happen throughout the school day and across a child’s entire school journey. A transition might be moving from one activity to another, joining a new class, or starting at a new school.

Supporting transitions is a shared effort. Students, families, educators, and community partners each play a role in helping these moments feel predictable, and connected.

Reporting Incidents

We are committed to creating learning environments where every student feels safe, valued, and included. 

Report It
If you witness or experience discrimination related to disability or ableism, you can use the Report It form to share your concerns. This helps us respond, support students and families, and continue building schools where everyone belongs.
